School-Based Care Coordination
Program Description
Offers assistance to parents of children with complex emotional and behavioral health needs currently in seven Bridgeport schools who require coordination of community based services so that the child/youth may remain in their home, school and community. Five Care Coordinators work with families to identify their needs and arrange services for children and youth using both traditional and non-traditional services and coordinate the efforts of all providers in- order to maximize their functioning and maintaining young people in their homes and communities. Program Eligibility:
The program is available to any parent(s) with a child or adolescent up to the age 18 (or 21 if still in school) who require coordination of services due to complex emotional and behavioral needs that is registered in one of the seven identified Bridgeport schools (Barnum, Bridgeport Learning Center, Dunbar, Garfield, Harding, Marin, Wilbur Cross).

Client Fees:
There is no fee to the family for participating in School-based Care Coordination. This is a free service to those who meet the criteria.
