Program Description:
Enhanced Care Coordinators
provide assessments and discharge recommendations for children/youth that have been
placed in Residential care. The primary goal is to return children/youth to their
homes and communities whenever possible. The focus of this program may also include
assessments on children/youth that are at high risk of disrupting from the community
and may be placed in hospitals or shelters.

Program Eligibility:
The program is available for children/youth
who have been placed outside the home or are at high-risk of disrupting from their
home up to the age of 18.

All referrals are directed to the Program Director

Client Fees:
There is no fee for this service for those eligible.
