Reconnecting Families

 

Program Description:

 

This is a Department of Children’s and Families funded program operated by Child Guidance Center of Greater Bridgeport.  The service is designed to engage,

 

support and intervene with family members through a short term, intensive in home service model following the removal of a child(ren) due to abuse or neglect. 

 

The program consists of three phases, Preparation for Reunification, (60 days) Reunification Period, (30 days) and Post reunification (60 days). This program will

 

work towards successful reunification and reduce the risk for further abuse and neglect. 

Program Eligibility:

All referrals will be made by the Department of Children and Families area office.  The program is for families with children from birth to seventeen years of age

who were removed from their home due to protective service concerns and whom supervised visits are needed.  These children may be victims of neglect and

physical and sexual abuse and may have been exposed to substance abuse, domestic violence, mental health issues and/or other disabling conditions of the parent.

Accessing Services:

Visitation will be facilitated by therapeutic support staff at the agency and in the home.  The North Carolina Family Assessment (NCFAS-R) will be completed to

assess the parent’s readiness for reunification as well as any parent child concerns. Provider meetings will be held prior and during each phase as needed. 

Client Fees:

No fees.
